Finding the right home for an adult with IDD is one of the most important decisions a family or case manager will make. We take that seriously and we move at a pace that feels right for the person, not the paperwork.

Who is Turning Point Living right for?

Turning Point Living is designed for adults 18 and older with intellectual and developmental disabilities who need 24/7 residential support in a home setting. We welcome individuals with a range of support needs, including those who may have complex behavioral or medical histories.

We are a small home by design. This means we can offer a level of individualized attention and consistency that larger facilities cannot, but it also means we have limited capacity and take admissions thoughtfully.

Eligibility criteria

  • Adults 18+ with intellectual or developmental disabilities
  • Requires 24-hour around-the-clock care and supervision
  • Wisconsin resident eligible for Medicaid waiver or private pay
  • Referral from county case manager or DHS preferred
  • Family or guardian involvement in the process

How the process works

Here's what to expect from first contact to move-in.

Initial inquiry

Reach out by phone or email to start a conversation. We'll ask a few questions to understand the person's support needs and whether Turning Point Living might be a good fit.

Information sharing

We'll request relevant documentation including the individual's current care plan, assessment, and any medical or behavioral history to help us understand their needs.

Home visit

We invite prospective residents and their families to visit Turning Point Living, meet the team, and see the environment. This is a two-way conversation. We want to make sure it's the right fit for everyone.

Admission decision

If we're a good match, we'll work with the individual's case manager and DHS to complete the admission process. We move at a pace that feels right for the person and their family.

What we'll ask for and why

We ask for documentation not to create barriers, but to make sure we can genuinely serve the person well. The more we understand about someone's history, preferences, and support needs before they arrive, the better we can prepare.

We don't require everything upfront. We'll work with you to gather what's needed at a pace that makes sense. If you're early in the process and don't have everything yet, that's fine. Start the conversation and we'll figure it out together.

Helpful documents

  • Current person-centered care plan or ISP
  • Recent functional assessment or level-of-care determination
  • Medical history and current medication list
  • Behavioral support plan (if applicable)
  • Contact information for current case manager
  • Any previous residential placement history
